ABSTRACT:
Background: Mullerian anomalies are congenital abnormalities affecting the development of the female reproductive tract, including the uterus, cervix, fallopian tubes, and upper vagina. These conditions may lead to absolute uterine factor infertility (AUFI), where women are unable to conceive or carry a pregnancy. Uterus transplantation has emerged as a novel and advanced procedure in reproductive medicine aimed at restoring reproductive function in such women. Objectives: To describe the importance, procedural steps, and clinical significance of uterus transplantation in restoring the ability to carry a pregnancy among women with uterine factor infertility. Methods: The article outlines a detailed procedural approach to uterus transplantation, including program setup, candidate selection, donor and recipient evaluation, surgical retrieval and implantation, immunosuppressive management, rejection surveillance, and pregnancy management. The procedure involves multidisciplinary coordination, advanced surgical techniques, and assisted reproductive technologies such as in vitro fertilization (IVF). Results: Uterus transplantation has demonstrated the potential to enable women with AUFI to achieve pregnancy and childbirth. The procedure improves psychological well-being, provides an alternative to surrogacy, and represents a significant advancement in reproductive medicine. Successful cases of pregnancy and live births have been reported, although the procedure remains complex and associated with risks such as graft rejection, surgical complications, and the need for immunosuppressive therapy.
